The question
A 67-year-old man had uneventful phacoemulsification four months ago. Since then he has had repeated mild redness and blurring that improve on topical steroids but return each time they are tapered. Now there are large mutton-fat keratic precipitates, a few vitreous cells and a white plaque on the posterior capsule inside the capsular bag. An intravitreal vancomycin injection a month ago helped only briefly. What is the next best step?
